超音波下什麼時候要懷疑abnormal metastatic lymph nodes

 超音波特徵

• Enlargement 
• Loss of the fatty hilum (OR 1.9) 
• 圓形而非橢圓形( 長軸/短軸) <= 2  (OR 1.6) 
• Hyperechogenicity (OR 5.4) 
• Cystic change (OR 71.8)
• Calcification  (OR 6.2)
• 周邊不正常血流 (OR 3.8)

位置

• 相較於Level II ，Metastatic nodule比較常出現在Level III,  VI 。
• 但，位於 upper pole 之PTC 也容易在 level II, III 看到metastasis。

懷疑有metastatic lymph node可以做 FNAC- Tg washout 

• 診斷沒有明確threshold。
• 如果thyroid gland還在(尤其是central LN) 可能false positive。
